Message from the HOD
It gives me a great pleasure to welcome you to our Hospital. We are a dynamic institution with state-of-the-art facilities and extremely dedicated physicians who provide patient care at par with international standards. We accomplish the above as a team that includes consultants, fellows, residents, nurses and the rest of the support staff. Our residents are an integral part of what we do and contribute immensely to our institution by their dedication, hard work and excellent work ethics.
These qualities, in addition to a sound fund of knowledge, are extremely important in a good physician. Here at SKMCH&RC we strive to impart these qualities in our trainees. In addition to teaching by personal example, a special emphasis is placed on the medical education of our residents. The education is provided in various forms that include daily didactics, teaching rounds and direct patient care. We are committed to train our residents to become excellent physicians and contribute to the field of medicine with the skills they have learnt during their time spent with us.
While we continue to strive to make it better, I take great pride and joy in our residency programme and can confidently say that once our residents complete their training with us, they are well-rounded physicians capable of providing best care to those in need.
Message from the Programme Director
Welcome to the IM Residency Training Programme. At Shaukat Khanum Memorial Cancer Hospital and Research Center, we aim to train our residents to become excellent physicians and patient advocates. Here, we will arm you with skills to diagnose and treat your patients using international guidelines. You will have the assistance of latest technology that helps in carrying out advanced diagnostic procedures that give a better understanding of the disease processes. You will interact closely with the consultants, fellows and support personnel not only from your own department but also from other departments. This multi-disciplinary approach ensures provision of excellent care to our patients.
While training you in the field of medicine is of utmost importance, equally important is our focus on your growth in the field of research as well as personal development. We encourage our residents to develop analytical thinking that generates questions, which our residents attempt to answer by performing clinical trials. Our faculty will mentor you in this process starting with hypothesis generation and will be a constant source of knowledge and support while you conduct and publish your research. This process is facilitated by the availability of electronic records of our patients that can be accessed for information. Our residents have presented their work in a number of conferences, both nationally and internationally like ACCP and ATS meeting in the United States. Currently, the department has a number of on-going research projects that our residents are welcome to be a part of.
I can say with utmost confidence that after training with us, you will become a competent, well-rounded physician who has the knowledge and the capability to treat complex medical issues and provide excellent patient care.
I look forward to having you join us to become a part of the IM family here at Shaukat Khanum Memorial Cancer Hospital and Research Centre.
The residency programme runs under the direct supervision of the IM Residency Programme Director (PD). The PD ensures smooth functioning of the residency maintaining the quality of the didactics as well as addressing concerns of the residents. The PD is available to the residents at all times.
Two Chief Residents are nominated each year to serve as a liaison between the IM faculty and the residents and being the senior residents, are an excellent resource for their junior colleagues.
During your training here, you will have daytime duties as well as night calls. As you gain experience, you will be assigned to areas with increasing responsibilities like the Emergency Room and Intensive Care Unit (ICU). Post call residents sign out to their team members and go home earlier in the afternoon.
Two month rotations in various specialties are offered. You rotate through the core Internal Medicine rotation in addition to subspecialty rotations including Critical Care Medicine, Emergency Medicine, Gastroenterology and Hepatology, Infectious Diseases, Medical Oncology and Pulmonology as well as a two month rotation in Cardiology at the Punjab Institute of Cardiology.
You interact with the consultants on service on a one-to-one basis on daily rounds as well as in the outpatient clinics. This ensures proper patient care as well as continued teaching.
While in your first year, you work as a junior resident and are paired with a senior colleague on rotations. This allows you to gradually ease into the system as well as learn the basics of the disease. The residents rotate through all the ICU, Emergency Assessment Room (EAR) and medical oncology both as a junior and senior resident. This gives them a chance to gradually build their skills and knowledge base in these important areas of medicine.
Excellent exposure to cardiology is offered during the two month external rotation at the Punjab Institute of Cardiology.
Two months of away electives are offered in the second year of training to enable to residents to work in a specialty of their choice in order to gain exposure. The residents are responsible to arrange the electives themselves. Final approval of the place of elective is given by the Programme Director.
A two month rotation, known as a “Day Float”, is offered where in the residents are available to help their colleagues in need for assistance. When not busy, the day float resident avails the time working on research as well as reading.
Rotations in diagnostic imaging as well as subspecialty clinics are also offered to enhance the experience and strengthen the knowledge base of our residents.
Daily resident-led and consultant-supervised lectures are given to the residents from 8:00 am to 9:00 am. The topics include some common and some rare diseases seen across the board as well as MKSAP questions. This provides an opportunity to discuss questions and problems regarding patient diagnosis and management in an open and supportive environment.
Thursday Morning Report: Every Thursday, the ICU residents present an interesting patient and lead the discussion generating a differential diagnosis followed by literature review on the selected topic. This helps residents improve their critical thinking in the diagnosis and management of critically-ill patients.
Interesting Cases of the Month: Thursday afternoons are reserved for interesting cases seen by the residents during the month. The case discussions stem from a variety of inpatient and ambulatory settings with an emphasis on developing clinical reasoning skills. The residents go over a variety of CT scans and laboratory reports as aids to the history provided to help generate a diagnosis. The conference alternates between pulmonary, GI and IM cases on a week to week basis.
Physical Exam Series: On alternate Fridays, the fellows lead the residents in going over the physical examination on selected patients to help prepare for TOACS and short and long cases in the FCPS exams.
Weekly Journal Club: A journal club is held every week where the residents go over a selected article published in an indexed journal. The residents learn the skills involved in critically appraising the literature to answer clinical questions, hypothesis generation and interpretation of statistics as well as review current advances in the field of medicine. The seminars also cover skills needed to practice Evidence-Based Medicine including medical informatics, how to keep up-to-date, and complex medical decision-making.
Infectious Diseases Conference: A consultant-led conference on an interesting case seen by the Infectious Diseases team is held where the residents go over the history and physical exam and are led through interpretation of data and pertinent investigations before coming up with a diagnosis and treatment plan.
Continuing Medical Education (CME): Twice a week we have a one hour CME presented by the consultants where latest literature on a wide variety of topics is presented and discussed.
Morbidity and Mortality Conferences: Held monthly, it reviews various “near misses” and deaths. This generates a critical event review and assesses the system-based issues with coming up with recommendations for improvement.
Research: As an IM resident here, you will have tremendous opportunities to work on on-going research projects as well as start your own. We have dedicated faculty with extensive research experience and publications in renowned world class journals. These faculty members are always available to mentor the residents with hypothesis generation, protocol writing followed by carrying out research and manuscript writing. In the past our residents have presented their work in conferences both nationally as well as internationally and are encouraged to do so. We have an electronic data base of more than 100,000 patients along with dedicated statisticians and epidemiologist in our research office available to help you.
