Referring Physicians

You can refer suspected or diagnosed cancer patients to one of the facilities of Shaukat Khanum Memorial Trust. The information provided by you will be used only for the purpose of your referral and according to our privacy policy.

Cancer patients are triaged based on hospital guidelines. A feedback will be provided to you following referral.

Information needed for referral

You will be required to send us relevant information in order to proceed with the referral including:

• Referring physician name and contact information
• Patient’s name
• Patient’s date of birth
• Patient’s address and phone number
• Patient diagnosis and treatment history
