GUIDELINES FOR BOOKING AN APPOINTMENT AT CHCC

1. You can make a provisional appointment for CHCC by calling above mentioned contact numbers from Monday through Sunday (8AM to 10 PM). To confirm this appointment, kindly deposit a sum of Rs. 5,000 as advance payment. The payment can be made at the OPD counter or online (Click Here).
2. In case you wish to cancel/reschedule your appointment, please intimate at least 48 hours prior to scheduled appointment, so the advance payment can be adjusted to rescheduled appointment or refunded. If we do not receive intimation for rescheduling/cancellation 48 hours prior to the scheduled appointment, the advance payment made would be forfeited.
3. In case of corporate clients, Authorisation Letter issued by the employer will be required to confirm an appointment.

VISIT 1: (FIRST APPOINTMENT)

• Upon reaching the OPD desk, kindly get yourself registered and proceed with the payment.
• OPD staff will guide you to the Consultation Room.
• After consultation and examination, chest X-ray and ultrasound* will be done.
• For patients above 40 years of age:
  • During consultation, the physician will assess if CT Coronary Angiogram is indicated.
  • If CT Coronary angiogram is not indicated, below tests will be refunded:
    • CT Coronary Calcium Scoring for CHCC
    • CT Coronary Angiogram for CHCC
  • If CT Coronary Angiogram is indicated:
    • You will proceed for CT Coronary Calcium Scoring.
    • Based on Calcium Scoring, you will be communicated if CT Coronary Angiogram will be performed.
    • If CT Coronary Angiogram is not being performed, below test will be refunded:
      • CT Coronary Angiogram for CHCC
    • If CT Coronary Angiogram is to be performed, appointment will be made and a document ‘CT Coronary Angiogram Information/Instructions for Patients’ will be shared with you, containing necessary preparatory instructions.
  • For blood tests next morning [fasting], you can give your samples at the nearest Shaukat Khanum Collection Centre** or through home sample collection (with no service charge).

Note:

*For ultrasound, at least 4 hours of fasting is required

VISIT 2: (FOLLOW-UP APPOINTMENT)

• You will visit for CT Coronary Angiogram (after taking preparatory steps) as per scheduled appointment.
• In case CT Coronary Angiogram cannot be done because of clinical reasons, a repeat appointment can be scheduled. If same result prevails in repeat appointment, and you do not wish to have the test done, following test may be refunded:
  • CT Coronary Angiogram for CHCC

VISIT 3: (FOLLOW-UP APPOINTMENT)

• On the follow-up visit, either in person or through telemedicine as per your convenience, the same consultant will discuss your health status with you, in view of the reports of diagnostic tests.
• In case any potential health issue is identified, further plan of action will be discussed in detail.
• Consultation with Clinical Nutritionist, as opted, for in-person visit or telemedicine clinic will follow.
• Clinical Summary will be handed over to you, along with complete set of reports of the tests performed.

CHCC PACKAGES

*Pap Smear will be conducted with no additional charges if clinically indicated.
